Induction Order Form

• Hard copy signed form to be mailed prior to the induction date

Honorary Member Form

One form required for each honorary member to be inducted

• Include a paragraph explanation for how the honorary inductee embodies the AΣN tenets
• Due via email SIX weeks prior to the induction date
• Hard copy signed forms to be mailed with Induction Order Form prior to the inductiond date

• Complete the information exactly as the example row indicates
• Alphabetize all inductees, including the honorary inductees, by last name
• Due via email SIX weeks prior to the induction date
• No hard copy is necessary
